Health Promotion Action Coalition

The Health Promotion Action Coalition (HPAC), Inc. is a 501(c) (3) nonprofit organization. Since its establishment in 2011, HPAC is dedicated to improving individual and community health outcomes while reducing the burden of chronic health diseases through collaborations with health advocates, community based organizations and local health departments. We have partnered with a number of community stakeholders and agencies representing diverse constituencies to reduce health disparities, promote health equity and to influence those modifiable social determinants of health critical to positive public health within our communities. Our model of collaboration focuses on the development of innovative partnerships that seek to build community agency and capacity to support the promotion of positive public health initiatives that strengthen the environment where our constituents live.

HPAC, has been recognized as a change agent and advocate for improving health outcomes in communities throughout Fulton County. As an example, HPAC presently serves as a partner and the community arm for the Fulton County’s Department of Health and Wellness Partnerships to Improve Community Health (PICH) grant that is funded by the Centers for Disease Control and Prevention. PICH supports initiatives that promote “implementation of evidence-based strategies to improve the health of communities and reduce the prevalence of chronic disease.” In 2015, HPAC began implementing programs focused on three key areas of the PICH initiative: increasing physical activity, improving nutritional habits and behaviors, and increasing the number of tobacco smoke-free environments.

Walk 4 Wellness (W4W)

Being physically active is a key component to sustaining a healthy lifestyle. HPAC recognizes the value of physical activity and has established walking clubs with diverse houses of worship, apartment communities, or other community organizations in our Walk 4 Wellness program. Designed for all ages, HPAC leverages our “walking club handbook” to guide new and existing walking clubs in achieving their physical activity goals and enhance healthy outcomes.

Walk 4 Wellness (W4W)

Pitch & Play (PN’P)

Childhood obesity remains a serious issue in Fulton County, Georgia and nationally. Georgia ranks 43rd in the nation for children’s health and well-being. Additionally, more than 237,000 uninsured children have limited access to regular primary health care. Children deserve opportunities for optimal health and development to be healthy physically and emotionally to position them to thrive developmentally.  HPAC strives to support such opportunities through our PITCH & Play initiative. This program promotes children’s positive physical and cognitive development by modeling healthy behaviors for children between 2 and 5 years of age and their parents. The program is fun and encourages physical activity through games, healthy eating and good nutrition, and reduced screen time.

Secondhand Smoke Exposure Prevention And Awareness (SHSEPA)

HPAC works with apartment complex owners, property managers and county residents to address the impact of secondhand smoke exposure. Our team of community advocates and public health practitioners is led by experts from Georgia State University and the Georgia Department of Public Health – Tobacco Smoke-Free Department. We provide informational sessions for apartment residents and staff to raise awareness in support of community efforts to become smoke-free. We also engage other community and state resources in support of these efforts.

Better Health Exchange

HPAC provides activities that educate, promote, and reinforce positive individual health behaviors. Our health education program: “Be Fit. Be Well. Be Whole – Better Health Exchange (BHE)” was designed to inform, engage and support faith leaders to start and/or enhance their health ministries while promoting healthier lifestyles that produce better health outcomes. BHE provides a platform to bring people together, increase awareness and leverage community resources to prevent and mitigate chronic diseases, and ameliorate health disparities. The BHE equips people with the tools to process and understand basic health information and services needed to make appropriate health decisions, and to better serve their respective congregations and communities.
